Friday, June 14, 2013 Campaign for Tobacco-Free Kids Wants to Tie Childhood Education to Future High Levels of Smoking; Undermines National Tobacco Control Funding In a destructive move that will have negative repercussions for future funding of tobacco control programs, the Campaign for Tobacco-Free Kids has turned its back on the state-of-the-art in tobacco control and instead, is pushing for a cigarette tax increase that will further reduce incentives for the government to reduce cigarette use. In a letter to the editor published in the New York Times, the Campaign for Tobacco-Free Kids supports President Obama's proposal to increase the federal cigarette excise tax by 94 cents in order to fund early childhood education programs. The revenues would be used to help expand pre-school programs. None of this money would be allocated for tobacco control or even other health-related programs. The Rest of the Story The state-of-the-art in tobacco control is the use of cigarette tax increases to fund comprehensive tobacco control programs. This is precisely what California did in 1989, and this is specifically why California has been so successful in reducing smoking prevalence. Unfortunately, this model is not being used any more, largely because the Campaign for Tobacco-Free Kids turned its back on this model. Instead, the Campaign decided to, in knee-jerk reflex fashion, support any and all cigarette tax increases, regardless of their underlying purpose or the use of the resulting revenue. Why is it that so few states are visit the site funding their anti-smoking programs at adequate levels? Part of the answer, believe it or not, is the actions of the Campaign for Tobacco-Free Kids. In the late 1990s, the Campaign made a decision to support increasing state cigarette taxes without tying those tax increases to smoking-related spending. The Campaign initiated campaigns throughout the country to increase cigarette taxes, but did not insist that the revenues be allocated for treatment of smoking-related diseases, research to prevent or cure smoking-related diseases, or anti-smoking education and prevention programs. As a result, numerous tax increases were enacted with no tie to smoking-related programs. Thus, the public learned to dissociate the two. The public also learned to distrust policy makers because they failed to see the tobacco revenues being used for smoking-related purposes. They saw the revenues simply being plopped into the general fund. Largely because of this, it is now very difficult to pass any cigarette tax increase as the public trust is not there. Moreover, the idea of using cigarette taxes for smoking programs has all but disappeared. It is my belief other that in no small way, the Campaign for Tobacco-Free Kids is responsible for the devastation of statewide tobacco control in the United States. The low allocation of state budget resources - especially cigarette tax revenues - for anti-smoking programs is very much the Campaign for Tobacco-Free Kids' doing. In addition to undermining funding for tobacco control programs, this policy has the disastrous effect of permanently tying v2cigs coupon code 15 the provision of pre-school education to continued high rates of cigarette smoking. In the future, what would happen if smoking rates dramatically drop? The answer is that pre-school education programs would have to be cut because of declines in cigarette tax revenue. Essentially, this proposal would allow smokers to boast that it is thanks to them that our nation's 4 year-olds are being educated. Can you imagine bumper stickers like: "Support Pre-School Education: Smoke Early and Often," or "I Support Kids: I Smoke." This is how perverse the effects of the proposed tax would be. Should the Congress increase the cigarette tax? Well, it depends on the purpose of the tax increase. If the money is used to fund pre-school education, then no. It makes no sense, it makes this essential program dependent on continued high rates of smoking, it removes the incentive for the government to substantially reduce smoking rates, it undermines the principle of spending tobacco revenues on tobacco control, and it is unfair to smokers to have to shoulder the burden of funding pre-kindergarten education programs. But if the money were to be used specifically for anti-smoking programs, then it would truly become a win-win situation. The tax would not only decrease cigarette consumption but would provide much-needed funding for tobacco control. It would be fair to smokers, because much of the revenue would be used to directly benefit smokers, such as finding more effective treatments for smoking-related diseases and providing specific services to smokers. Moreover, if the revenues decline over time, the program needs less money because there are fewer smokers. Nicotine Increases Exercise Endurance16/7/2013

Thursday, March 21, 2013 Nicotine Increases Exercise Endurance In my lectures on tobacco harm reduction I compare the properties of nicotine with those of caffeine (see slides at left).  Despite some obvious differences, the drugs have remarkably similar effects.  I have just found a study from 2006 showing that ??nicotine administration during moderate-intensity exercise delays fatigue, with a significant improvement of 17% [±7%] in time to exhaustion. This observation is similar to observations of the effects of caffeine supplementation.? (Available at the journal Experimental Physiology here ). Authors Toby M?ndel and David A. Jones of the Human Performance Laboratory, School of Sport and Exercise Sciences, The University of Birmingham (United Kingdom), recruited 12 healthy non-smoking men and asked them to cycle in a laboratory setting at a moderate pace until exhausted on two occasions.  Subjects randomly applied either a 7 milligram nicotine patch or a placebo hyperlink patch the evening before.   Ten subjects who wore the nicotine patch cycled for 70 minutes ? about 17% longer than the 62 minutes cycled by those with the placebo patch.  Nicotine had no effect on heart rate or respiratory parameters, and it ??did not alter the perception of effort? associated with progressive fatigue.?  The researchers noted that ??activity of dopamine pathways has been suggested to be sites associated with improved endurance exercise performance.?  In other words, nicotine?s endurance boost stemmed from its effect on the brain.  As I mentioned in 2011 ( here ), the World Anti-Doping Agency (WADA) is considering labeling nicotine a performance-enhancing drug.  However, WADA could treat nicotine as it has caffeine, summarized this way by the agency in 2012 ( here ): ?Caffeine was removed from the Prohibited List in 2004. Its use in sport is not prohibited.  Many experts believe that caffeine is ubiquitous in beverages and food and that reducing the threshold might therefore create the risk of sanctioning athletes for social or diet consumption of caffeine.  In addition, caffeine is metabolized at very different rates in individuals.? Since using nicotine is a ?social? choice and the substance is metabolized at very different rates, one can only hope that WADA applies such a reasoned, practical analysis to it as well. Posted by

Thursday, June 20, 2013 Alternative Tobacco Products Aid in Smoking Cessation In May Dr. Lucy Popova and Dr. Pamela Ling coauthored ?Alternative Tobacco Product Use and Smoking Cessation: A National Study.?  It appeared in the May issue http://WhiteSmokeReview.com cartimizer of the American Journal of Public Health (abstract here ). The authors are members of the Center for Tobacco Control Research and Education at the University of California, San Francisco, which is a staunch opponent of tobacco harm reduction.  So it is unsurprising that their conclusions about safer smoke-free products were almost entirely negative: ?Alternative tobacco products are attractive to smokers who want to quit smoking, but these data did not indicate that alternative tobacco products promote cessation.  Unsubstantiated overt and implied claims that alternative tobacco products aid smoking cessation should be prohibited.? But these data DID indicate that alternative products promote cessation.  The authors used a Knowledge Network survey in 2011 of 1,836 electronic cigarette reviews consumer reports current or former smokers who view quit less than two years before.  The survey asked participants about use of chewing tobacco, moist snuff, snus, dissolvable tobacco and e-cigarettes.  Drs. Lorillard eCigs leading in market share

It has launched an estimated $40 million marketing campaign for the devices. Tuesday, May 14, 2013 EU politician shows what governments really fear about e-cigarettes Giancarlo Scottà, a member of the European Parliament from Italy, recently posted this question to his fellow EU  politicians: "The consumption of traditional cigarettes provides the Member States with sizeable revenues, as a result of the substantial taxes to which they are subject.  According to a recent report by ANSA (Italian news agency) of 21 April 2013, in the first two months of 2013 alone, Italy?s coffers registered a loss of EUR 132 million, corresponding to a fall in revenue from duty on tobacco of approximately 7.6%. Of course, this shortfall cannot be completely blamed on the increasing use of electronic cigarettes, but it is certainly partly responsible.   In light of the above, can the Council state what action it intends to take to address the differences in tax revenue materialising in State coffers following the proliferation of electronic cigarettes, which ebay e cigarettes currently appear to be free from any form of duty?" E-cigarettes: the bad news is... people are buying fewer cigarettes. Mr. Scottà's comment clearly shows a concern that increased e-cigarette use will have a huge impact on tax revenues and he strongly hints that taxes on e-cigarettes will be the solution. There is no doubt that this same concern has been expressed in hushed tones and behind closed doors in the hallowed halls of government all across the U.S., as well. The first reaction of the average citizen is to disregard "conspiracy theories" about our great leaders' willingness to put tax revenues before public health, but Mr. Scottà's comment is proof positive that it isn't just a theory that our governments' opposition to electronic cigarettes is likely to be, in large part, financial. It certainly isn't based on scientific evidence of harm or any other justification for excessive "sin taxes" to be applied to electronic cigarettes. It may not be a conspiracy, but our government shouldn't be more concerned about lost revenue than it is about the well-being of 45 million of its citizens. The recent attempts by several U.S. states to add sin taxes (different from the sales tax applied to other nicotine products) to electronic cigarette liquid is more evidence that it isn't about our health. Mind you, the ANTZ (Anti Nicotine and Tobacco Zealots) and legislators attempt to justify high sin taxes on cigarettes for two main reasons: 1) the products ostensibly cause health problems for which other taxpayers end up paying and 2) higher taxes means higher cost, which (theoretically) will cause more smokers to quit. However, electronic cigarette use has not been shown to cause health problems and the lower costs associated with vaping actually encourages smokers to quit smoking and switch to a relatively safe product. The only reason legislators have at this time to tax the product differently is to make up future lost cigarette tax revenue, even while doing so would clearly have the opposite effect intended for applying a sin tax to a "harmful" product! Obviously, increasing the cost of electronic cigarettes (and other smoke-free alternatives) would eliminate a huge incentive for smokers to switch to e-cigarettes. The only people who would benefit from that are ANTZ, tobacco companies, pharmaceutical companies and politicians. People keep smoking and the sales profits, tax revenues and "research" and "smoking cessation" funding keep rolling in. However, as a product which is not likely to increase health care costs to taxpayers (indeed, it is more likely to reduce such costs) and actually encourages quitting smoking, e-cigarettes should not be subject to anything greater than a standard sales tax - the same as any other nicotine product on the market. Clearly, e-cigarettes have thrown a monkey wrench in the tax revenue gears. Although the ANTZ can honestly claim smoking rates are going down, the number of smokers has changed little over the past two decades - 46 million U.S. smokers in 1990 vs. 45 million smokers today - even as cigarette taxes have increased more than 1,000-fold in some areas. Yet, as populations have increased, ANTZ could still claim their "evidence-based" smoking cessation methods were working, ensuring that their future funding would continue. Governments were secure in the knowledge that the numbers weren't really going down and the tax revenue would keep coming in. Pharmaceutical companies could keep selling their snake oil smoking "cures." They never dreamt that something would come along that actually could significantly reduce the number of cigarette packs being sold! Now that there is, they are running scared. It's not a conspiracy theory, it's just simple arithmetic. Smokers switching to e-cigarettes means less tax revenue - good for smokers, bad for governments addicted to their cigarette taxes.
