Research shows that patients who lack the skills and confidence to manage their own health care often require more of it and incur higher health care costs. "Patients who are actively engaged in their own health care are more likely to stay healthy and manage their conditions," said Susan Mende, senior program officer at RWJF. "It is important for physicians and their teams to work with patients to ensure they have the knowledge and confidence necessary to take control of their own healthwhere most health decisions happenoutside of the examination room." The RWJF package highlights how health care professionals across the country are engaging patients at all levels to improve care, from hosting disease self-management workshops to including patients on practice quality improvement teams. "Patient engagement isn't only about patients' ability to engage in their own health or quality improvement projects," said Jessica Osborne-Safsnes, project co-director of Aligning Forces Humboldt , Calif. "It's also about how ready an organization is to work with patients on these projects." The materials also include a resource guide and interview with Safsnes and a colleague about their successful patient engagement efforts. The resources are part of the Aligning Forces for Quality "Quality Field Notes" series highlighting lessons learned by regional alliances of clinicians, patients, and payers to transform local health care and provide models for national reform.

Researchers saw a 47 percent increase in those who reported weekly acid reflux symptoms by the end of the study. Acid reflux occurs when stomach acid touches the lining of the esophagus and causes a burning sensation in the throat and chest, also known as heartburn. Consuming acidic or fried and fatty foods can bring it on. While the study does not mention why acid reflux is on the rise, the researchers suggest that the increasing numbers may be linked to a rise in obesity rates. "The interabdominal pressure that goes along with being obese allows for more reflux," said Dr. Gerard Mullin, associate professor of medicine at Johns Hopkins University, who was not involved in the Gut study. "It does seem like it's paralleling the prevalence of obesity." Cigarette smoking and stress can also lead to acid reflux, Mullin said.

Cause Doctors and researchers do not know the cause of irritable bowel syndrome, but some believe the condition results from intestinal sensitivity, with flareups resulting from eating habits and stress factors. Some evidence points to the presence of bacteria in the intestinal tract and that this may cause or instigate flareups of the condition. In addition, some people with irritable bowel syndrome may have a condition known as celiac disease, resulting from a reaction to gluten, a component of grains such as wheat and barley. It can occur for example, in people who sustain severe burns and are thus hospitalised for long periods of time. While the precise mechanism by which this occurs remains a matter for scientific research to define clearly, that association is now beyond any debate. So also does it occur in people who sustain head injuries from road traffic accidents, sports accidents and from falling from heights. It is known to be associated with these situations because of the stress the individuals who suffer them undergo hence they are truly stress ulcers in every sense of that word. Peptic ulcer also has factors known to make it worse such as chronic liver or kidney disease, the consumption of large amounts of alcohol and the indulgence in heavy smoking. It used to be believed that a lot of thinking and the eating of certain types of food were the major causes of these diseases but that is no longer the case. It is wise for those who suffer from it to see their doctor to identify the cause. Peptic ulcer sometimes becomes complicated. These complications range from perforation to bleeding from the ulcer site. Perforation is an emergency which is characterised by severe abdominal pain, progressive abdominal distension and a diminishing ability to breathe properly.

What makes Raspberry Ketone Ultra work for many is that aside from just the main ingredient, raspberry ketone, it is also added with African Mango. The ingredient has also been clinically proven to promote a natural and fast weight loss experience. I was a bit skeptical at first after having tried raspberry ketone from a different manufacturer whose product caused me to be bloated/gassy, have horrible acid reflux etc, & I actually gained weight. So I then purchased this product. I don't own a scale, but I can tell you, after a few weeks of taking this product, my clothing no longer fits me tight around my waist & legs. I seem to have more energy. I haven't been bloated & nauseous. I'm not eating as much & it's helped me to become regular & I haven't experienced any acid reflux either. This is a product that you have to be patient with. I eventually plan to add exercise & change my diet as well, but for now, I am very happy with this product & have just ordered my 2nd bottle. "There are little things here and there [that pop up], especially at restaurants," she said. "People don't understand that anything with gluten that you put on top of something thats gluten-free - it can affect me." Castronova, 56, is one of 3 million Americans with celiac disease, an autoimmune digestive disorder caused by gluten protein, found in wheat, rye and barley. Celiac disease is diagnosed with a specific blood test that looks for the presence of antibodies to gluten, and with an endoscopy that allows for a biopsy of the intestine. Doctors first diagnosed Castronova in 1961 at age 4, and then again in 1989. Following the birth of her second child in 1987, she started having massive flare-ups. Her weight dropped to 63 pounds on her 5-foot-fall frame. Her weight is back to a normal range but Castronova still struggles with a constant arthritic-like soreness, listlessness, and ongoing stomach pain caused by damage to her small intestine. "You learn to live with it," she said. "It becomes your new normal." Castronova said she hopes new therapies will soon emerge for people with celiac disease.

The participants' average number of steps taken per day was recorded at the start of the programs and again 12 months later. Amounts of walking at the start of the programs and changes in amounts of walking over 12 months affected the participants' risk of heart disease, according to the study, which was published Dec. 19 in the journal The Lancet. For every 2,000 steps more per day a person took at the start of the study, they had a 10 percent lower risk for heart disease in subsequent years. And for every 2,000 steps per day increase during the study period, the risk of heart disease fell an additional 8 percent, the researchers found. For example, if Person A took 4,000 steps per day at the start of the study and did not change that amount over the next year, and Person B took 6,000 steps per day at the start of the study and increased to 8,000 steps per day during the next year, Person B would have an 18 percent lower risk of heart disease than Person A by the end of the study, the researchers said. "Our results provide novel evidence that changing physical-activity levels through simply increasing the number of steps taken can substantially reduce the risk of [heart] disease," study leader Dr. Thomas Yates, of the University of Leicester, in England, said in a journal news release. "Importantly, these benefits are seen regardless of body weight or the starting level of activity," Yates said.

"Checking four to six times per day during the holidays is a good idea, keeping in mind that the frequency might even be higher depending on your child's blood sugar readings." Kashmiri noted that too many restrictions may lead children to sneak food, which can be dangerous. "There is a misconception that a child with diabetes has to avoid sweets. That's not true," he said. "Children with diabetes just need insulin to help them process the food." It's important that your children know they need to tell you if they are eating certain foods so you can give them an appropriate amount of insulin, Kashmiri said. "If you keep the communication lines open and help the child know you are on the same team, a child will be less likely to sneak snacks, which can cause extreme elevations in blood sugars," he added. "You'll want to closely monitor blood sugar, but also make sure they can have fun." More information The U.S. National Library of Medicine has more about diabetes in children and teens . Recent reports estimate that the eye disease affects about 745,000 people with type 2 diabetes in the United States, the authors noted in background information. The people in the survey with diabetic macular edema responded to questions about their medical care. The Johns Hopkins researchers gleaned their findings from the survey responses.

With help from Mordecai, I put together this list of five things you need to know about GERD and esophageal cancer. Here they are: Though esophageal cancer is rare, its incidence is rising (according to ECAN, its incidence has risen by 400 percent in the past 20 years), for reasons that are poorly understood. While most people with GERD will not develop esophageal cancer, GERD remains a major risk factor for that cancer. Common symptoms of GERD include hoarseness, cough, throat clearing, regurgitation, sore throat and wheezing. Controlling GERD symptoms with medications doesn't necessarily protect against esophageal cancer. And not everyone who has GERD experiences recognizable symptoms. According to the American Cancer Society, five-year survival rates for those diagnosed with esophageal cancer are quite low, ranging from 17 percent to 37 percent, depending on how localized the cancer is. The ACS notes that "Cancer of the esophagus usually is diagnosed at a late stage and therefore has a poor outlook for survival." There is debate in the medical community as to who should be screened for esophageal cancer. Talk with your doctor about your own risk and whether screening is appropriate. Here's more information about esophageal cancer.

Over time, stomach acid in the esophagus can cause changes in the cells that increase risk for adenocarcinoma. In addition, certain groups -- men, the elderly, and African-Americans -- are at greater risk for esophageal cancer. How Is Esophageal Cancer Diagnosed? To diagnose esophageal cancer, your doctor will review your symptoms, medical history, and examine you. In addition, he or she may order certain blood tests and X-rays. Tests for esophageal cancer may include: Barium swallow X-ray, in which you drink a liquid that coats your esophagus. This makes the esophagus stand out on the X-ray so that your doctor can identify certain problems. Endoscopy: the doctor passes an endoscope, a thin, lighted tube, down your throat into your esophagus to examine it. Endoscopic ultrasound uses sound waves to provide more information about the extent of tumor involvement in nearby tissues. Biopsy: during an endoscopy, the doctor can take cells or tissue from your esophagus.
